   Note: For a corporate account, application should be made by an authorised officer (Manager and above) on its behalf and a photocopy of its business registration certificate should be attached to the form. Incomplete forms will not be processed.

Payment can be in the form of cheque, money order or bank draft made payable to "Axac Solutions Pte Ltd". Please include your name and contact number overleaf. For cash payment, please proceed to our office during our business hours. All such payments must be in Singapore Dollars only.
